The OSHP Pharmacy Stars Shining Star Compounding Pharmacy Technician Award

  Nomination Form

Purpose

To recognize an individual who has demonstrated dedication to patient safety and advancing the role of pharmacy technicians in Ohio.

Criteria

The nominee must be a current pharmacy technician whose daily tasks are focused on compounding. Board members are not eligible for awards during their term of service to OSHP.

Nominations

May be submitted along with supporting materials(s) by any non-OSHP individuals, any OSHP member, OSHP Board of Directors, or Local Affiliated Chapters of OSHP.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.

June 6, 2025
