Todd W. Nesbit, PharmD, MBA, CPEL, FASHP
Chief Pharmacy Officer , The Johns Hopkins Hospital, Vice President for Pharmacy Services, Johns Hopkins Health System

Todd W. Nesbit graduated from The Ohio State University Doctor of Pharmacy Program in 1992 and from the Johns Hopkins University Carey Business School in 2008. Dr. Nesbit has worked to promote and advance the role of the pharmacist and clinical pharmacy in health-systems throughout his career. He currently serves as Chief Pharmacy Officer for The Johns Hopkins Hospital and Vice President for Pharmacy Services for the Johns Hopkins Health System. As Executive Leader for the pharmacy enterprise, he is responsible for directing pharmacy services across the continuum.

Todd began his career in 1984 following graduation from Ohio Northern University with a Bachelor of Science degree in pharmacy. He serves as the Residency Program Director for the Johns Hopkins Hospital Pharmacy Administration and Leadership Residency Program. He has been active in state and national pharmacy organizations and has been recognized as a Fellow and has achieved the status of Certified Pharmacy Executive Leader by the American Society of Health-System Pharmacists (ASHP). He currently serves on the ASHP Board of Directors.


Dr. Megan Adelman, BCPS, BCGP is a Family Medicine Pharmacy Specialist at the Cleveland Clinic Akron General Center for Family Medicine. She earned her Doctor of Pharmacy from the Northeast Ohio Medical University and completed her PGY1 Pharmacy Residency at the Carolinas Medical Center and a PGY2 in Geriatrics at the Louis Stokes Cleveland VAMC. Her practice and research interests include generational learning, gaming in education, weight and metabolic health management, geriatric care, polypharmacy and deprescribing, and population health management. 

Colleen Cloyd, PharmD, BCPPS completed her PharmD at the University of California, San Francisco and residency at Lucile Packard Children’s Hospital in Palo Alto, California. She is currently a clinical pharmacist in the pediatric intensive care unit at Nationwide Children’s Hospital in Columbus, Ohio. Her specific areas of interest include quality improvement, anticoagulation, and pediatric obesity. In the last several years she has been a part of multiple department and hospital-wide protocol developments and EHR improvements, including the one presented today. Outside of her career Colleen enjoys cooking, baking and cake decorating, sideline sitting with her husband at her kids’ soccer and baseball games, exercising, and traveling. 

Dr. Amy Gillman is a board-certified critical care clinical pharmacist at OhioHealth Grant Medical Center in Columbus, OH. She completed her PGY1 residency training at Kettering Health Dayton (formerly Grandview Medical Center) in Dayton, OH and her PGY2 to specialize in critical care at the University Medical Center of Southern Nevada in Las Vegas, NV. Her practice interests include everything trauma and neurocritical care and supporting the mental health of her student and resident learners. After her own experience as a patient in 2024, she is more committed than ever to optimizing patient care as well as the patient experience.

Jacob Holmes, PharmD, is a current PGY1 Pharmacy Resident at Mercy Health – West Hospital in Cincinnati, OH. He earned his Doctor of Pharmacy degree from the University of Findlay. Through his residency, Jacob will gain comprehensive, hands-on clinical experience across a variety of clinical practice settings, including Internal Medicine, Antimicrobial Stewardship, Critical Care, Emergency Medicine, and more. His research project focuses on optimizing concomitant antiplatelet use in warfarin therapy to improve patient safety and therapeutic outcomes.
In addition to clinical activities, Jacob is passionate about academic pharmacy and fostering growth in future pharmacists. Jacob is pursuing his teaching certificate through the University of Cincinnati. After completing residency, Jacob plans to obtain board certification and practice as a clinical specialist or pursue a professor role at an accredited University.

Peter Hopwood, PharmD, is a PGY-2 Ambulatory Care Resident at The MetroHealth System in Cleveland, Ohio. His professional interests include primary care, heart failure, cardiometabolic disease, and expanding the role of pharmacists through clinical practice and research.

Lindsay Kalata, PharmD is a Clinical Informatics Pharmacist at Nationwide Children's Hospital in Columbus, Ohio. She earned her PharmD from West Virginia University School of Pharmacy and completed a Drug Information Specialty Residency. Dr. Kalata has specialized in pharmacy informatics since 2017, focusing on clinical decision support, medication safety, and workflow optimization. She is passionate about leveraging technology to improve medication-use processes and patient care.

Justin Kullgren, Pharm.D., FAAHPM, FPPCP is a nationally recognized clinical pharmacy specialist in pain and palliative care, currently serving at The Ohio State University Wexner Medical Center. With two decades of experience spanning clinical practice, academia, and professional leadership, Dr. Kullgren has pioneered interdisciplinary approaches to symptom management, opioid stewardship, and harm reduction in seriously ill populations. He is a Fellow of both the American Academy of Hospice and Palliative Medicine and the Society of Pain and Palliative Care Pharmacists, where he also served as President. A prolific educator and researcher, he has authored numerous peer-reviewed publications, delivered over 75 invited presentations, and mentored students, residents, and fellows across multiple institutions. His work continues to shape the evolving landscape of palliative pharmacy practice. 

Victoria Lubarsky, RPh, MBA, BCACP is a Pharmacy Manager at University Hospitals Cleveland Medical Center in Cleveland, Ohio, where she leads system-wide transitions of care (TOC) pharmacy services and inpatient pharmacy operations. She is responsible for overseeing multidisciplinary initiatives that improve medication safety, streamline admission and discharge workflows, impact hospital readmissions, and expand access to care through programs such as Meds-to-Beds, telehealth follow-up, and population health services. Ms. Lubarsky is recognized for her expertise in transitions of care, value-based care delivery, medication safety, formulary and 340B program optimization, and pharmacy operations leadership across inpatient and outpatient settings.

She earned her Master of Business Administration from the University of Derby, England and her Bachelor of Science in Pharmacy from the Hebrew University School of Pharmacy in Jerusalem, Israel. She is a Board-Certified Ambulatory Care Pharmacist and holds additional training in process improvement and innovation. Ms. Lubarsky is an Assistant Professor of Practice Management at Northeast Ohio Medical University (NEOMED) and serves as a preceptor for pharmacy learners. She is an active member of professional organizations and was recognized as the 2025 Health-System Pharmacist of the Year, reflecting her leadership, innovation, and impact on advancing pharmacy practice.

Meighan Marlo, PharmD, is a Clinical Pharmacist in Gastroenterology and Nutrition Support at Nationwide Children's Hospital in Columbus, Ohio. She earned her PharmD  from Southern Illinois University Edwardsville and completed both PGY-1 and PGY-2 residency training at Nationwide Children’s Hospital. Her professional interests center on the care of patients undergoing intestinal rehabilitation and on advancing the clinical effectiveness and safety of total parenteral nutrition in the pediatric population. As a member of the multidisciplinary team that redesigned home parenteral nutrition prescribing at Nationwide Children’s Hospital, she has contributed to the initiative’s practical implementation into patient care and helped lead training for providers and pharmacists.

Jordan Millin is a Clinical Lead Pharmacist in Internal Medicine and Transitions of Care at Summa Health System in Akron, Ohio, as well as an Assistant Professor of Pharmacy Practice at Northeast Ohio Medical University (NEOMED) in Rootstown, OH.

Jordan earned her Bachelor of Science in Pharmaceutics Sciences/Bioethics Minor from The Ohio State University in 2019 and Doctor of Pharmacy degree from The Ohio State University in 2023. She completed a PGY1 Pharmacy Practice Residency at Summa Health from 2023-2024. She has been a Board-Certified Pharmacotherapy Specialist since September 2025, and her professional interests include Internal Medicine, Transitions of Care, and Academia.

Kristian Navickas, PharmD, BCACP is the Site Manager of Ambulatory Pharmacy Services at OhioHealth, where she supports anticoagulation, antiarrhythmic, cardiometabolic, and other ambulatory pharmacy services. She leads clinical and operational initiatives focused on advancing patient care, optimizing outcomes, and supporting pharmacy teams across ambulatory practice settings. 

Dr. Navickas also serves as the PGY2 Ambulatory Care Pharmacy Residency Program Director at Riverside Methodist Hospital. Her professional interests include ambulatory care practice advancement, disease management, leadership development, and resident education and mentorship.

Nathan J. Oblizajek, PharmD, MHA, BCPS, is Manager of Oncology Infusion Pharmacy Services at The James Cancer Hospital and Solove Research Institute and The Ohio State University Wexner Medical Center (OSUWMC). Nathan completed his PharmD training at Ohio Northern University. He subsequently completed the combined PGY1/PGY2 MHA Health-System Pharmacy Administration and Leadership Residency at West Virginia University Hospitals in Morgantown, West Virginia.
At OSUWMC, Nathan helps oversee pharmacy operations across outpatient oncology infusion pharmacy sites, with responsibilities including staffing management, workflow optimization, regulatory compliance, and coordination of pharmacy services across multiple infusion locations.
Nathan’s current practice interests include pharmacy operations, medication safety, and oncology services.

Andrea Pallotta is the clinical coordinator for Infectious Diseases and HIV at Cleveland Clinic Main Campus and Antimicrobial Stewardship Lead for the Cleveland Clinic Health System. She works alongside a multidisciplinary team of caregivers to provide treatment for patients living with HIV in outpatient clinic and to coordinate antimicrobial stewardship efforts across the enterprise. Andrea practices in HIV prevention (PrEP) clinic and provides comprehensive HIV and STD prevention services.

Dr. Stephanie Parton earned her Doctor of Pharmacy degree from the University of Kentucky College of Pharmacy and completed a PGY1 Pharmacy Residency and a PGY2 Internal Medicine Pharmacy Residency at TriHealth - Good Samaritan Hospital in Cincinnati, OH. She is a board-certified pharmacotherapy specialist and works as an Internal Medicine Clinical Pharmacy Specialist at UC Health - University of Cincinnati Medical Center in Cincinnati, OH. In addition, she serves as the PGY2 Internal Medicine Assistant Residency Program Director.

Dr. Mary Petrovskis, PharmD, MS, BCPS is the Associate Director of Pharmacy at University Hospital – East Hospital, Ohio State Wexner Medical Center’s community hospital and Talbot Hall, a comprehensive substance use disorder treatment center. She also has oversight for OSUWMC controlled substance and diversion prevention programs. Mary received her Bachelors and Doctor of Pharmacy Degrees from The Ohio State University. She completed a two-year residency in Health-System Pharmacy Administration at UNC Hospitals and a Master of Science Degree in Health-System Pharmacy at UNC Eshelman School of Pharmacy.

She is active in professional organizations, and her professional interests include medication safety, technology and automation, controlled substance and diversion prevention, employee engagement, and patient access to care. Outside of work Mary enjoys spending time exploring with her family, gardening and cooking.

Stephen R. Polley, PharmD, MPA, MS, is Director, Cancer Pharmacy Services at The James and The Ohio State University Wexner Medical Center (OSUWMC). Stephen completed his PharmD training and Master of Public Administration at the University of Kentucky. Stephen completed the combined PGY1/PGY2 MS in Health-System Pharmacy Administration Residency at The University of Kansas Health System in Kansas City, Kansas.

At OSUWMC, Stephen oversees pharmacy services for OSU's eight oncology infusion pharmacies, pain and palliative, Opioid Stewardship, Investigational Drug Service and James Medication Access. Stephen’s current practice interests include pharmacy operations, revenue cycle management, and oncology services.

Isabella Popielarczyk, PharmD, MS is the Pharmacy Manager for Distribution Services at University Hospitals Cleveland Medical Center in Cleveland, Ohio, where she provides operational leadership for inpatient pharmacy distribution services supporting approximately one million medication doses annually and oversees a team of nearly 80 pharmacy professionals.

Dr. Popielarczyk earned her Doctor of Pharmacy degree from the University of Toledo College of Pharmacy and a Master of Science in Pharmaceutical Sciences with a specialization in Health System Pharmacy Administration from the University of North Carolina Eshelman School of Pharmacy. She completed both PGY-1 Pharmacy Practice and PGY-2 Health-System Pharmacy Administration and Leadership residencies at University Hospitals Cleveland Medical Center. 

Dr. Popielarczyk has presented regionally and nationally on topics including telepharmacy, pharmacy governance, and operational leadership and is proud to serve on the American Society of Health-System Pharmacists (ASHP) Section of Pharmacy Practice Leaders Leadership Development and Succession Planning Workgroup among others. She was recognized as the Ohio Society of Health-System Pharmacists PGY2 Resident of the Year in 2025.

James E. Reissig, PharmD, MS, BCPS, is the Clinical Pharmacy Manager at University Hospitals Cleveland Medical Center, where he oversees adult inpatient clinical pharmacy services and leads a team of Clinical Pharmacy Specialists. Throughout more than 25 years of health-system pharmacy practice, he has been dedicated to advancing clinical pharmacy services, developing pharmacy leaders, and improving patient care across a variety of practice settings. 

Dr. Reissig began his clinical career as a Pain Management and Palliative Care Clinical Pharmacy Specialist at Akron General Medical Center, where he provided expertise in symptom management, pain control, and medication optimization for complex patients. His work included extensive interdisciplinary collaboration, clinician and patient education, resident precepting, and contributions to the medical literature, including publication in the Annals of Pharmacotherapy. Dr. Reissig earned his Doctor of Pharmacy degree from The University of Toledo and a Master of Science in Health-System Pharmacy Administration from Northeastern Ohio Medical University. He is a Board-Certified Pharmacotherapy Specialist. Outside of work, he enjoys golfing, hiking, tennis, and spending time outdoors.

Jacalyn Rogers, PharmD, MS, CHM, BCPS, CPEL, FASHP is Senior Director of Pharmacy for the Academic Market at University Hospitals in Cleveland, Ohio. She oversees pharmacy services across multiple hospitals and cancer centers, leads pharmacy workforce and residency development initiatives, and is a recognized leader in health-system pharmacy practice and innovation.

Dr. Ashley Scherreik is a board-certified pharmacist with experience in Internal Medicine across academic and community health systems and ambulatory care clinic. She earned her Doctor of Pharmacy degree from Cedarville University School of Pharmacy and completed a PGY1 Pharmacy Residency at Premier Health Miami Valley Hospital in Dayton, Ohio. She currently practices as an Internal Medicine Clinical Pharmacy Specialist and serves as the PGY2 Internal Medicine Residency Program Director at the University of Cincinnati Medical Center in Cincinnati, Ohio. She is passionate about learner development, preceptor development, and advancing pharmacy education and practice. As an Internal Medicine pharmacist at UC Health, she works closely with internal medicine physicians and hepatology consultants to manage patients with decompensated cirrhosis, pre-transplant evaluations, and post-transplant rejection cases.

Ashley Schuler, PharmD, BCCCP is a Neurocritical Care Pharmacist and PGY2 Critical Care Pharmacy Residency Coordinator at OhioHealth Riverside Methodist Hospital in Columbus, Ohio. She received her Doctor of Pharmacy degree from Regis University in Denver, CO and completed a PGY1/PGY2 in Critical Care residency at Denver Health in Denver, CO. Dr. Schuler’s scholarly work includes publications in the Journal of Pharmacy Practice and the Journal of Thrombosis and Thrombolysis. Her clinical and research interests center on optimizing anticoagulation/antiplatelet therapy, anti-epileptic therapy, and sedation management in neurocritical care patients.  

Dr. Erika Thomas graduated from the University of Pittsburgh School of Pharmacy in 2022 and went on to complete a community-based PGY1 residency with UH Meds. Following residency, she remained with UH Meds and helped expand the Meds to Beds program in collaboration with Ahuja Medical Center. Dr. Thomas currently serve as a Transitions of Care Pharmacist, working with both Ahuja Medical Center and Minoff Pharmacy.

R. Zachary Thompson, PharmD, MPH, BCPPS, is a Clinical Pharmacy Manager at Nationwide Children's Hospital in Columbus, Ohio. He earned his PharmD from the University of Kentucky and completed PGY1 and PGY2 residency training at UK HealthCare, followed by an MPH from The Ohio State University. His clinical background is in pediatric critical care and has been in clinical leadership for 9 years at Nationwide. He has interests in clinical operations, medication safety, and informatics, specifically focused on how to integrate the three to make more efficient and safe systems. He led the multidisciplinary redesign of home parenteral nutrition prescribing at Nationwide Children's, a fully EHR-based, transcription-free workflow spanning the inpatient and ambulatory settings.

Margaret (Peggy) Williams graduated from OSU College of Medicine in 2012 and completed internal medicine residency at the University of Michigan in 2015, followed by a Global Health Fellowship in Botswana and Boston through Beth Israel Deaconess Medical Center.  She joined OSUMC’s Division of Hospital Medicine in 2016 and founded the OSUMC inpatient addiction medicine consult team at the Main campus in 2018. She is board credentialed in internal medicine and addiction medicine.

Janet Wu is an Infectious Diseases Clinical Pharmacist at Cleveland Clinic Main Campus where she primarily practices in the HIV pre-exposure prophylaxis (PrEP) clinic. In this role, she provides PrEP patient care, education, and community outreach focused on HIV prevention. Janet is committed to improving access to HIV and sexually transmitted infection screening and advancing PrEP education initiatives throughout the Cleveland area.

Briana Wukovich, CPhT-Adv, CSPT, is an Advanced Certified Pharmacy Technician with over 16 years of experience across community, hospital, oncology, association management, and medication therapy management. She holds certifications in sterile compounding, controlled substance diversion prevention, hazardous drugs, supply chain management, immunization administration, and well-being and resilience. She currently serves as the Professional Development and Continuing Education Coordinator for the Ohio Pharmacists Association and works contingent as a Certified Pharmacy Technician Support Specialist with OhioHealth. Briana is also a graduate of NPTA’s Pharmacy Leadership and Management Bootcamp, PTCB’s Leadership Institute and a certified ADHD coach, advocating for neurodiversity within the pharmacy profession.

Connie H. Yoon, PharmD, BCPS, BCCCP, is a Neurocritical Care Clinical Pharmacist at OhioHealth Riverside Methodist Hospital in Columbus, Ohio, where she provides care for critically ill patients with acute neurologic and neurosurgical conditions. She also serves as a coordinator for the PGY2 Critical Care Pharmacy Residency Program and as an Associate Editor for Annals of Pharmacotherapy. Dr. Yoon received her Doctor of Pharmacy degree and completed both PGY1 Pharmacy Practice and PGY2 Pharmacotherapy residencies at the University of Maryland. Her professional interests include neurocritical care pharmacotherapy, status epilepticus, anticoagulation reversal, sedation and analgesia, and optimization of medication therapy in patients with acute neurologic injury. She is actively involved in research, resident education, and advancement of evidence-based neurocritical care practice.
