Purpose

To recognize contributions to and sustained support of OSHP from someone, not necessarily a pharmacist. This award was created in 1989 as the OSHP Recognition Award and it was renamed in honor of its first recipient, Richard Surface, in 1999.

Criteria

As determined by the OSHP Board of Directors to recognize sustained service to the Organization

Nomination

May be submitted along with supporting material(s) by any OSHP member, OSHP Board of Directors, or Local Affiliated Chapters of OSHP. Nomination proposals should be submitted to the OSHP Board of Directors.

Selection

Requires unanimous vote of the OSHP Board of Directors.
