Purpose 
To recognize an individual who has demonstrated sustained exemplary service in health-system and community service

Criteria 
The individual should be of high moral character, good citizenship, high professional ideals, and who has made significant contributions to health-system pharmacy

Nomination 
Nominations may be submitted by Frazier Award Committee members, the OSHP Board of Directors, or Local Affiliated Chapters of OSHP

Selection 
Made by the Frazier Award Committee (past Frazier Award recipients plus current members of the Board Directors)

Deadline 
June 6, 2025
