Purpose

To recognize a health-system pharmacist and Ohio Society of Health-System Pharmacists (OSHP) member of high integrity, high professional ideals, and who best exemplifies the profession of health-system pharmacy practice

Criteria

The focus of this award is based upon recent (within last 2-5 years) accomplishments to the profession of pharmacy. OSHP membership is a prerequisite.

Nomination

Nominations may be submitted by any OSHP member, OSHP Board of Directors, and Local Affiliated Chapters of OSHP

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.
