Awards: The OSHP Best Practice Advancement Success Award

Nomination Form I Previous Recipients

Purpose

To recognize innovative Ohio Health-Systems who have demonstrated pharmacy practice advancements in accordance with the American Society of Health-Systems Pharmacy (ASHP) Pharmacy Practice Advancement Initiative (PAI).

Criteria

Nominations must represent pharmacy departments located within the state of Ohio. Any Ohio institution is eligible, and this award is not limited to just pharmacy Health-Systems. Departments should establish an innovative best practice that has demonstrated clear benefits in patient care and the advancement of pharmacy practice. Emphasis should be placed on incorporating at least one of ASHP’s Pharmacy Practice Advancement Initiative Objectives into institutional practice. 

Award winning programs will be asked to create a video or PowerPoint showcasing their best practice initiatives. A focus should be placed on implementation and initiation of various phases of the project.

To be eligible for the award, nominations should include information regarding the institutions background, practice setting, and current pharmacy practice model. Additionally, institutions should highlight their innovative practice initiatives including outcomes and future directions. Finally, strategies for potential sustainment should be incorporated into the nomination.

Nomination

Nominations may be submitted along with supporting material(s) by any OSHP member, OSHP Board of Directors, or Local Affiliated Chapters of OSHP. Self-nominations are acceptable for this award. Use of an OSHP Practice Advancement Success Award Nomination form is required.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.

Deadline

June 30th

Presentation

The Best Practice Advancement Award presentation will be showcased at the OSHP Annual Awards Banquet. One individual from the institution is required to be in person at the OSHP Annual Meeting to present a slide set showcasing the project.
