The OSHP Resident Award
Nomination Form | Previous Recipients

Purpose
To recognize a pharmacy resident who has demonstrated exceptional skills in the area of residency practice.

Criteria
The nominee must currently be in, or have completed a residency program within the past year.

If appropriate, both a PGY-1 and a PGY-2 Resident Award will be presented at the discretion of the Professional Affairs Division and approved by the OSHP Board of Directors.

Nomination
May be submitted by any member of OSHP, OSHP Board of Directors, or Local Affiliated Chapters. A letter of recommendation from a pharmacy residency director or a program preceptor must accompany the nomination (maximum three letters).  

Selection
Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.

Deadline
January 15

Presentation
OSHP Annual Meeting.

Award
Plaque; Complimentary OSHP Annual Meeting Registration; Complimentary one-year OSHP membership.
