The OSHP Health-System Pharmacist of the Year Award

Purpose
To recognize a health-system pharmacist and OSHP member of high integrity, high professional ideals, and whom best exemplifies the profession of health-system pharmacy practice.

Criteria
The focus of this award is based upon recent accomplishments to the profession, preferably within the least year, although this is not an absolute period. OSHP membership is a prerequisite.

Nomination
Nominations may be submitted by any OSHP member, OSHP Board of Directors, and Local Affiliated Chapters of OSHP. Use of a Health-System Pharmacist of the Year Nomination Form is required.

Selection
The OSHP Division of Professional Affairs will select the winner from available nominees. Final approval will be made by the OSHP Board of Directors.
