The OSHP Timothy D. Moore Pharmacy Management Award

Purpose
To recognize the health-system pharmacist who has demonstrated significant managerial performance in the delivery of health-system pharmacy services in a health-system organization.

Criteria
The focus of this award is based upon recent managerial expertise and leadership which resulted in significant improvement or expansion of pharmacy services and concomitant management of available resources.

Nomination
Nominations may be submitted by any OSHP member, OSHP Board of Directors, and Local Affiliated Chapters of OSHP. Use of an OSHP Health System Pharmacy Management Award Nomination form is required.

NOTE: Due to the nature of this award (i.e., recognition as a health-system pharmacy manager), recommendation by the candidate's administrative supervisor, Director of Pharmacy (if applicable) is strongly encouraged. In addition, letters of endorsement from pertinent Health-system Administrative individuals (i.e., Nursing Administration,Medical staff, etc) should be solicited and submitted. Written letters of endorsement should be attached to this nomination form or sent under a separate cover letter to the Chairman, Professional Affairs Division.

Selection
The OSHP Division of Professional Affairs will select the winner from available nominees. Final approval will be made by the OSHP Board of Directors.
