Purpose
To recognize an individual and/or organization who/which has made outstanding effort/progress toward furthering medication safety.

Criteria
This award is based upon recent (preferably within the last year) accomplishments. This award will recognize effort by an individual or by a health-system organization that has made a positive impact on medication safety through system controls, educational programs, implementation of safe practices/processes, encouraging the reporting of medication errors, or other methods. Where possible, objective data will be reviewed and evaluated.

Nominations
Nominations may be submitted by any OSHP member. Use of the on-line nomination form is required. Supportive documents and letters of recommendation by supervisory personnel may be submitted in support of the nomination.

Selection
The Professional Affairs Division award selects the award winner from among the pool of nominees. The OSHP Board of Directors will make final approval of the selection.
