Purpose
To recognize the health-system pharmacist that contributed most to society in general over the past year.

Criteria
The health-system pharmacist should demonstrate humanitarian ideals.

Nominations
Nominations may be submitted by any OSHP member. Any supporting material should be submitted with this form.

SelectionThe OSHP Division of Professional Affairs will select the winner from available nominees. Final approval will be made by the OSHP Board of Directors.
