The OSHP Walter M. Frazier Award

Purpose
To recognize an individual who has demonstrated sustained exemplary service in health-system and community service.

Criteria
The individual should be of high moral character, good citizenship, and high professional ideals, who has made significant contributions to health-system pharmacy.

Nomination
Nominations may be submitted by Frazier Award Committee members, the OSHP Board of Directors, Local Affiliated Chapters of OSHP, or any three members of the OSHP. Use of an OSHP Walter M. Frazier Award Nomination Form is required for nomination.

Selection
Selection of the award winner is made by the Frazier Award Committee (past Frazier Award recipients plus current members of the Board Directors).
